APE Data Form & Attachments

The ACGME Common Program Requirements, stipulate that “the educational effectiveness of a program must be evaluated at least annually in a systematic manner." This is accomplished though the Annual Program Evaluation (APE). A Program Evaluation Committee (PEC) should be created to ensure that each program has undertaken and documented a systematic review process, on at least an annual basis.

APEs will now be completed on New Innovations. Program Directors and Administrators should receive an email when the APE form is published and available to complete. AY24-25 APEs are due on September 19. 2025.

Reminders:

  • Citations and Areas for Improvement must be added to New Innovations (Administration > Program > Citations & AFI/Concerns)

  • Action Plans must be added to New Innovations (Administration > Program > Action Plans)

  • The SWOT analysis must be completed within the PEC meeting minutes. It is no longer a separate attachment.

  • PEC minutes must be attached to the meeting date in the “Meetings” section.

  • Read each question carefully and ensure you are attaching the necessary supporting documents when prompted.
