Add a New Participating Site to Your Program

Add a New Participating Site - Currently Listed in WEBADS

1 - Your Request to Add a New Participating Site must be submitted to GMEC, by email, to Dr. Odinet Frey

    • send email to rodine@lsuhsc.edu
    • email to include
      • the name of the new site
      • the # of months per year that trainees rotate at the new site
      • the educational rationale

2 - Once received, Dr. Frey will present your Request to Add a New Participating Site, to GMEC

3 - If approved, you will be notified by GMEC

4 - Once notified, you MUST initiate the request in WEBADS

5 - Approval of your new participating site will be forwarded by WEBADS to Dr. Frey for approval

6 - Once approved, your RRC reviews the Request to Add a New Participating Site and provides confirmation to you via email

Add a New Participating Site - Not Currently Listed in WEBADS

1 - Your Request to Add a New Participating Site must be submitted to GMEC, by email, to Dr. Odinet Frey

        • None

        • First Name
        • Middle Initial
        • Last Name

2 - Once the new Institutional Site is added to WEBADS, complete your Request to Add a New Participating Site (currently existing in WEBADS)

3 - Submit your Request to Add a New Participating Site to GMEC, by emailing Dr. Frey

      • the name of the new site
      • the # of months per year that trainees rotate at the new site
      • the educational rationale

4 - Once received, Dr. Frey will present your site request, to GMEC

5 - If approved, you will be notified by GMEC

6 - Once notified, you MUST initiate the request in WEBADS

7 - Approval of your new participating site is forwarded by WEBADS to Dr. Frey for signature

8 - Once approved, your RRC reviews the site request and provides confirmation to you via email
