Required by the LSBME to inform them which residents will be active in your program for the upcoming academic year.

Instructions for Completing Form

Download completed letters for each resident from GME Online Appointment Forms.  Letters must be signed by the Program Director and then submitted with the LSBME Permit Renewal Spreadsheet to the GME Office.

Program Contact Information displayed in the letter comes from the contact information attached to the Program Director as specified in New Innovations.  Any changes can be made to the Office Address and Office Phone entries attached to the Program Director's Personnel Data.  Changes made will be reflected on the letter the following day.


Incoming House Officers - LSBME Program Director Letter:

Download completed letters for each incoming resident from the GME Online Appointment Forms.  Letters must be signed by the Program Director and then FedEx to the LSBME, address mailing to:   LSBME 630 Camp Street, New Orlenans LA 70130;  or address to the Attention of                Mr. Matthew Cook, Licensiing Analyst Supervisor, 630 Camp Street, New Orleans, LA 7030.
