For programs which rotate to OLOL:
All of the forms and documents below should be submitted to the GME Office. The GME Office will be providing all of these documents to OLOL on your behalf. For incoming House Officers, the new rotator forms are available for download with the other New Hire Documents. For current residents, the prepopulated forms are available for you to download at the Academic Year Forms site (https://www.medschool.lsuhsc.edu/medical_education/graduate/appointments/) on the “Resources” tab. I have attached the OLOL New Rotator Checklist and the Returning Resident Checklist for your reference, but more detailed information is contained here.

Each program must submit a signed House Staff Teaching Program Letter, listing all residents expected to rotate during the academic year. The prepopulated letter may be downloaded from the Academic Year Forms site (https://www.medschool.lsuhsc.edu/medical_education/graduate/appointments/) on the “Other Resources” tab. This signed letter should be submitted to OLOL directly at SMOfficeofMedicalEducation@fmolhs.org. |

For the GME Data Sheets, OLOL has requested that we deliver them before May 1st. So, PLEASE do everything you possibly can to get those to the GME Office ASAP. If you do not have the entire LSU New Hire Packet completed, you can send Chris a copy of the GME Data Sheet directly before submitting the rest of the New Hire Packet.
All OLOL forms (for new rotators and returning rotators) should all be submitted no later than 1 month before the House Officer’s first rotation at OLOL. (Submit to LSU New Orleans GME Office) These forms may be submitted on paper or electronically.

Our Lady of the Lake Health has an online application that must be completed through MSOW. Academic Affairs will send you information about completing this application from Academics@fmolhs.org.
In addition to the online application in MSOW, the following items will be requested by Academic Affairs. Please have these readily available when requested:
- Current Curriculum Vita or Resume
- Malpractice Insurance (LSU provides to Academic Affairs)
- Proof of current TB Skin test (must be within the past 12 months) and Flu Vaccination (seasonal)
- Documentation of compliance with your Institution’s COVID-19 Vaccination Policy
- Copy of Medical School diploma
- Professional Photo for ID Badge (jpeg format)
