For programs which rotate to OLOL:

All of the forms and documents below should be submitted to the GME Office.  The GME Office will be providing all of these documents to OLOL on your behalf.  For incoming House Officers, the new rotator forms are available for download with the other New Hire Documents.  For current residents, the prepopulated forms are available for you to download at the Academic Year Forms site (https://www.medschool.lsuhsc.edu/medical_education/graduate/appointments/) on the “Resources” tab.  I have attached the OLOL New Rotator Checklist and the Returning Resident Checklist for your reference, but more detailed information is contained here.

Forms Required for Each House Officer

Forms Required for New Rotators
(incoming residents or current residents who have never rotated to OLOL)

These forms must be completed by the new rotating House Officer and returned to the LSU New Orleans GME Office

  • OLOL House Officer Questionnaire
  • OLOL Rotator Acknowledgement Agreement Form
  • FMOLHS Information Security and Confidentiality Agreement
  • Resident Headshot for Badge (in JPG format) – This should be submitted directly to OLOL by you or the house officer.

Forms Required for Returning Rotators

This form should be completed by the House Officer and returned to the LSU GME Office

Additional Requirements

These should be submitted to the LSU GME Office if not already on file.

  • ECFMG Certificate (if applicable)
  • 2019 TB Test
  • Medical School Diploma
  • Curriculum Vitae  NOTE:  The CV is not something we typically have in our files for our house officers, so you will most likely need to submit one for all rotating residents.

Additional Requirements for Each Program

Each program must submit a signed House Staff Teaching Program Letter, listing all residents expected to rotate during the academic year.  The prepopulated letter may be downloaded from the Academic Year Forms site (https://www.medschool.lsuhsc.edu/medical_education/graduate/appointments/) on the “Other Resources” tab.  This signed letter should be submitted to OLOL directly at SMOfficeofMedicalEducation@fmolhs.org.

Deadlines and Submission

For the GME Data Sheets, OLOL has requested that we deliver them before May 1st.  So, PLEASE do everything you possibly can to get those to the GME Office ASAP.  If you do not have the entire LSU New Hire Packet completed, you can send Chris a copy of the GME Data Sheet directly before submitting the rest of the New Hire Packet.

All OLOL forms (for new rotators and returning rotators) should all be submitted no later than 1 month before the House Officer’s first rotation at OLOL.  (Submit to LSU New Orleans GME Office)  These forms may be submitted on paper or electronically.

All files submitted to us will be immediately forwarded to Our Lady of the Lake.  You may check to see what forms/documents are outstanding for your House Officers by going to the GME Resident Files site(https://ome-ccalla-serv.master.lsuhsc.edu/gme_files/) and clicking on the “Required Files by Rotation Site” link at the top of the page.  All house officers in your program who are expected to rotate to OLOL will be shown, and you will be shown the documents which have been received already and which ones are outstanding.  If there are residents you expect to rotate to OLOL who are not listed, contact the GME Office.
