Download completed letters for each resident from GME Online Appointment Forms. Letters must be signed by the Program Director and then submitted with the LSBME Permit Renewal Spreadsheet to the GME Office.
Incoming House Officers - LSBME Program Director Letter:

Letter must be signed by the Program Director and then submitted to the LSBME at: licensing@lsbme.la.gov If sending via FedEx, address to Mr. Matthew Cook. Program Contact Information displayed in the letter comes from the contact information attached to the Program Director as specified in New Innovations. Any changes can be made to the Office Address and Office Phone entries attached to the Program Director's Personnel Data. Changes made will be reflected on the letter the following day.
