OLOL Credentialling - May 1st Deadline - New and Returning Residents

Coordinators,

OLOL has requested that we get the needed credentialing forms to them by May 1st.  While I realize this may be challenging, they have been working with me to eliminate ALL of the manual processes and reduce the paper involved in the process, and I believe this process will ultimately be easier and require less effort from everyone.  If you foresee any problems with this, please contact me.


Current Residents who will rotate to OLOL during the 2018-2019 Academic Year (N.O. Programs):

  • The only thing required will be a signed copy of the OLOL Information Security and Confidentiality Agreement. Each resident can download their own personalized form using this link:  https://www.medschool.lsuhsc.edu/medical_education/graduate/appointments/getdocument.aspx?document=OLOL (will require LSU login, and forms are only available for HOs expected to rotate to OLOL during AY18-19).
  • Each resident will need to fill in any missing information (but most should already be populated), sign and date the form, and return it to you.
  • You will need to send completed forms to Chris (ccalla@lsuhsc.edu) or Stephanie (sgalen@lsuhsc.edu) in the GME Office. You can either send us the paper forms, or scan and send them to us.
  • The GME Office will submit the forms to OLOL.

New Hire Residents who will rotate to OLOL during the 2018-2019 Academic Year (N.O. Programs):

  • The LSU GME office will need both the Information Security and Confidentiality Agreement (available for New Hires with their other LSU and hospital packets), and the LSU GME Data Sheet.
  • Both the OLOL form and completed GME data sheet need to be submitted to Chris (ccalla@lsuhsc.edu) or Stephanie (sgalen@lsuhsc.edu) in the GME Office. You can either send us the paper forms, or scan and send them to us.
  • The LSU GME Office will submit the forms to OLOL.
  • Since the LSU GME Data Sheet will most likely be submitted to the GME Office before the remainder of the New Hire Packet, the GME Data Sheet will NOT need to be re-submitted with the New Hire Packet. Once the GME Office has received the GME Data Sheet, an updated New Hire Checklist will be available for you to download.  Please submit this updated checklist (which will indicate that we already have the GME Data Sheet).


Baton Rouge Programs:

  • You will need to do the same thing as the N.O. programs, except you can deliver your Confidentiality Agreements and GME Data Sheets directly to the OLOL GME Office. The N.O. GME Office will only need copies of the LSU GME Data Sheet (but you are welcome to send us the OLOL forms as well, it wouldn’t hurt for us to have copies in our files).
