COVID 19 Vaccination Policy
The deadline for submitting COVID vaccination status is December 5th 2021. Documentation from LA Wallet is fine, but it must be a screenshot instead of the “card” it produces. Anyone who answers “No” to being fully vaccinated in this portal form will be prompted to download, complete, and submit our exemption form.
