The QI Project Submission Form is used to submit all quality improvement projects on which LSUSOM faculty, staff and House Officers are participating. The form defines whether a project is ACTIVE active, completed or COMPLETEinactive, the project aims, numeric goals and time frames. Also identified is which patient population/systems are affected, if a project involves a hospital committee, and if so, which committeedescription, proposed project end date, and metrics. The primary facility where the project takes place and any ICDs it may include. A project requires measurable endpoints anticipated milestones with dates that will assess whether or not it was a success. |
