Quality Improvement Submission Form

GME Terminology

The QI Project Submission Form is used to submit all quality improvement projects on which LSUSOM faculty, staff and House Officers are participating. The form defines whether a project is ACTIVE or COMPLETE, the project aims, numeric goals and time frames. Also identified is which patient population/systems are affected, if a project involves a hospital committee, and if so, which committee. The primary facility where the project takes place and any ICDs it may include. A project requires measurable endpoints that will assess whether or not it was a success.
