GME Online Appointment Forms are available at https://www.medschool.lsuhsc.edu/medical_education/graduate/appointments/

If prompted, log in with your LSU username and password.

After successful login, you will see a screen similar to this one.  Click on each tab to access the section's content.

All of the data for these forms comes from New Innovations.  Appointment dates can be corrected in the Training Record section of Personnel Data.

Any changes made in New Innovations will be reflected here the following day.
