Add a New Participating Site Currently Listed in WEBADS

1 - Your Request to Add a New Participating Site must be submitted to GMEC, by email, to Dr. Odinet Frey

    • send email to rodine@lsuhsc.edu
    • email to include
      • the name of the new site
      • the # of months per year that trainees rotate at the new site
      • the educational rationale

2 - Once received, Dr. Frey will present your Request to Add a New Participating Site, to GMEC

3 - Once approved, you will be notified by GMEC

4 - Once notified, you MUST initiate the request in WEBADS

5 - Approval of your new participating site is requested from Dr. Frey

6 - Once approved, your RRC reviews the Request to Add a New Participating Site and provides confirmation to you via email

Add a New Participating Site Not Currently Listed in WEBADS

1 - Your Request to Add a New Participating Site must be submitted to GMEC, by email, to Dr. Odinet Frey

      • Institution Name
      • Institution Address
        • Address Line 1
        • Address Line 2
        • Address Line 3
        • City
        • State
        • Zip Code
      • Website Address
      • Is the site a recognized healthcare entity by any of the following (check all that apply):
        • Joint Commission

        • Community Health Accreditation Program (CHAP)
        • Accreditation Commission for Health Care (ACHC)
        • DNV
        • State regulatory commission
        • Medicare Direct - CMS
        • HFAP
        • None
        • Other (provide name)
      • CEO/President/Director Information
        • First Name
        • Middle Initial
        • Last Name
        • Suffix
        • Degrees
        • Title
        • Phone Number
          • Extension
        • Fax Number
        • Email Address
      • the educational rationale
      • the # of months per year that trainees rotate at the new site

2 - Once the NEW INSTITUTIONAL SITE is added to WEBADS, follow the steps above for Requesting to Add a New Participating Site (currently existing in WEBADS)

Add an Existing WEBADS Site

1 - Your Request to Add a New Participating Site must be submitted to GMEC, by email, to Dr. Odinet Frey

      • the # of months per year that trainees rotate at the new site
      • the educational rationale

2 - Once received, Dr. Frey will present your Request to Add a New Participating Site, to GMEC

3 - Once approved, you will be notified by GMEC
