What Program Administrator Is Responsible For

New Innovations Data

·         House officer personnel file opened with all data entered and accurate

·         Social Security numbers and Birth City must be manually entered

New Hire Packet

Appointment Packet

  • Items individually initiated by GME directly to house officer

  • Ensure house officer completes all forms appropriately

  • Appointment “Packet” includes FCVS Release, LSBME Release, COVID/Immunization Release, HO Manual Acknowledgement, Health Requirements Worksheet (Immunization Information with attachments), Moonlighting Form (signed by house officer, PD, and Dept. Chair), TB test submitted to GME (tested within 3 months of start date).

Drug Testing

Medical License

  • Program Director Letter downloaded, signed, and sent to LSBME

  • Monitor granted licenses via Resident License Check system

  • Ensure CDS license is applied for

  • Ensure house officer enrolls in Medicare as an Ordering and Referring Provider (PECOS). Monitor residents via Resident Data Status to make sure registration is successful.

NPI

  • Get NPI from resident and enter into New Innovations

I-9

·         Ensure house officer completes section I of I-9 and ensure section II is completed with HRM team member

Contract

·         Initiate contract from GME Online Appointment Forms system

·         Ensure house officer, PD, and Dept. Chair sign prior to start date

Orientation and Onboarding

·         Ensure house officer completes all requirements in the Virtual Orientation Portal

·         Ensure house officer attends GME Onboarding Day

Lab Coats

  •  Provide fitting instructions to resident

  •  order lab coats in proper sizes

What House Officer Is Responsible For

New Hire Paperwork

  • New Hire Paperwork should be submitted to your program no later than May 1. Paperwork submitted after this date could result in a delay of the July 1 start date.  If you have any questions contact your program administrator.

Hospital Credentialing Packets

  • Available with the New Hire documents (if needed)

  • Follow Instructions for submitting Hospital Credentialing Packets

Academic Year Appointment Electronic Forms/Submissions - Due May 31

  • Sign House Officer Agreement (contract) - Initiated electronically by Program Administrator to your LSUHSC email

  • Take TB Test and submit TB Test reading to GME with supporting documents here

Appointment “Packet”

Electronically sent to House Officer's LSU email address.  To be initiate by GME Office on May 15 and due June 15

  • Complete all “packet” forms

I-9 e-Verify - Employment Eligibility Verification (USCIS I-9)

Electronically sent to House Officer's LSU email address. To be initiated May 15, must be completed by June 1

  • Follow Instructions for completing Section I

  • Bring accepted I-9 documents to Onboarding Days  - must be submitted in person  List of acceptable documents  I-9 Acceptable Documents

Drug Testing

  • Contact your Program Administrator to schedule the drug test.

  • Drug testing should be completed by May 1st.

  • Return completed Drug Test Authorization Form to the Program Administrator

  • Return completed and signed Post Job Offer Drug Testing Instructions to the Program Administrator - Read the Section 4 carefully, especially about certain CBD products

Background Check

  • Email will be sent to House Officers from LSU HRM by April 15

  • Follow Instructions for completing the information for the  Background check within 7 days of receiving email

Medical Permit and/or License - Issued by the Louisiana State Board of Medical Examiners (LSBME)

This should be done as soon as possible to have a permit/license will be granted before July1.  If not issued by July 1, internship or residency training could be delayed. 

  • Complete LSBME permit or full licensure application on the LSBME website in LaMed Dashboard

  • Complete all forms required per instructions (notarize if required)

  • Submit notarized affidavit if required due to answers on the Oath or Affirmation form

  • Complete Fingerprinting

  • Complete FCVS paperwork if required (talk to LSBME Analyst)

  • Pay invoice found in LaMed Dashboard

NPI

Medicare / PECOS

  • Medicare / PECOS enrollment is now completed as part of the Incoming House Officer Orientation. 

  • Incoming House Officers NOT attending orientation will need to enroll themselves.

Take LSU ID Photo

  • ID  Photos are taken at the LSU GME in-person Onboarding Day

  • House Officers that are on campus, have the option of taking their ID Photo in the HRM office beginning May 1, located in the LSUHSC Resource Building, 433 Bolivar St. 6th floor.

  • Photo ID Badge will be given to the House Officers a the LSU in-person Onboarding Day

Lab Coat

  • Get fitted for lab coat (per instructions from Program Coordinator)

Attend EPIC Training

EPIC Training will be Virtual.  Virtual Epic Training Modules will be assigned to each incoming Resident/Fellow in the beginning of June, and the Residents/Fellows are expected to complete the Virtual Epic Training Modules prior to June 30 in order to ensure full access to the system.

Attend LCMC or other hospital orientation

Thursday, June 25, 2026. University Medical Center New Orleans, 8:00 a.m. to 4:00p.m., Location TBD.

Attend program orientation

Complete all Virtual Orientation requirements

Attend GME Onboarding Day

Sign up for Onboarding Session in the Virtual Orientation Portal

June 23, 2026-June 24, 2026, Lions Building, 2020 Gravier St. Bring a copy of your ssc and I-9 documents.
