New Hire Packet
- Send resident new hire paperwork instructions to resident from GME Online Appointment Forms system.
- Complete Per 2
- Check paperwork for accuracy and submit to GME office with required supporting documents. See Newhire Checklist which can be downloaded from GME Online Appointment Forms system.

- Distribute Incoming House Officer Checklist
- I-9 completion for each resident. The I-9 will be initiated by the GME office and the coordinator is responsible for collecting appropriate verified documents to be uploaded to the HireRight system. Instructions on the Hiring Manager process are available.
- Items individually initiated by GME directly to house officer
- Submit to GME office when signed by resident, program director and department head
- See Sample Appointment Packet which lists the items initiated by GME
Drug Testing
- Schedule testing online with i3 Screen and provide forms to resident. Instructions are available Drug Testing- i3screen User Guide
Medical License
- Program Director Letter downloaded, signed, and sent to LSBME
- Monitor granted licenses via Resident License Check system
NPI
- Get NPI from resident and enter into New Innovations
Lab Coats
- Provide fitting instructions to resident
- order lab coats in proper sizes
Medicare/PECOS
- Enrollment will be initiated during orientation. Monitor residents via Resident Data Status to make sure registration is successful. For incoming House Officers who do not attend orientation, you can have them apply themselves, or contact the GME Office for a completed application to submit.

New Hire Paperwork
- New Hire Paperwork should be submitted to your program no later than May 1. Paperwork submitted after this date could result in a delay of the July 1 start date. If you have any questions contact your program coordinator.
Hospital Credentialing Packets
- Available with the New Hire documents (if needed)
- Follow Instructions for submitting Hospital Credentialing Packets
Academic Year Appointment Electronic Forms/Submissions - Due May 31
- Sign House Officer Agreement (contract) - Initiated electronically by Program Coordinator after March 21 -Check email
- Take TB Test or submit TB Test reading to Program Coordinator with supporting documents
New Hire Electronic Forms
Electronically sent to House Officer's LSU email address. To be initiate by GME Office on May 15 and due June 15
- Follow instructions for Electronic Forms/Submission
I-9 e-Verify - Employment Eligibility Verification (USCIS I-9)
Electronically sent to House Officer's LSU email address. To be initiated May 15, must be completed by June 1
- Follow Instructions for completing Section I
- Bring accepted I-9 documents to Program Coordinator - must be submitted in person
Drug Testing
- Contact your Program Coordinator to schedule the drug test.
- Drug testing should be completed by May 1st.
- Return completed Drug Test Authorization Form to the Program Coordinator
- Return completed and signed Post Job Offer Drug Testing Instructions to the Program Coordinator - Read the Section 4 carefully, especially about certain CBD products
Medical Permit and/or License - Issued by the Louisiana State Board of Medical Examiners (LSBME)
This should be done as soon as possible to have a permit/license will be granted before July1. If not issued by July 1, internship or residency training could be delayed.
- Complete LSBME permit or full licensure application on the LSBME website in LaMed Dashboard
- Complete all forms required per instructions (notarize if required)
- Submit notarized affidavit if required due to answers on the Oath or Affirmation form
- Complete Fingerprinting
- Complete FCVS paperwork if required (talk to LSBME Analyst)
- Pay invoice found in LaMed Dashboard
NPI
- Apply for NPI and provide number to coordinator
Medicare / PECOS
- Medicare / PECOS enrollment is now completed as part of the Incoming House Officer Orientation.
- Incoming House Officers NOT attending orientation will need to enroll themselves.
Take LSU ID Photo
- Can be taken in the HR office beginning May 1, at the LSUHSC Resource Building, 433 Bolivar St. 6th floor
- If not taken ahead of time, will be done at LSU GME In-person Orientation.
LSU Parking
- Only needed for programs based at UMCNO
- Complete online at https://lsuhscnolaparking.t2hosted.com/cmn/index.aspx on in person at Parking Office (433 Bolivar St. 1st floor).
Lab Coat
- Get fitted for lab coat (per instructions from Program Coordinator)
Attend EPIC Training
EPIC Training will be Virtual. Virtual Epic Training Modules will be assigned to each incoming Resident/Fellow in the beginning of June, and the Residents/Fellows are expected to complete the Virtual Epic Training Modules prior to June 29, 2024, in order to ensure full access to the system.
Attend LCMC or other hospital orientation
Wednesday, June 26, 2024. University Medical Center New Orleans, 8:00 a.m. to 4:00p.m., Location TBD.
Attend program orientation
Attend GME Onboarding Day
- Monday, June 24, 2024, or Tuesday, June 25, 2024, 9:00 a.m. to 4:00 p.m., Lions Building, 2020 Gravier St, 6th Floor Lobby.
- Bring a copy of the Social Security Card (if not already submitted with new hire paperwork) for the house officer and all dependents as required for State of Louisiana Medical Insurance.
