PIV Card

Form Name:	PIV Card (OR VA PIV Card)
Issued By:	VA Medical Center
Used by:	All residents who rotate to the VA Medical Center and associated clinics.
Purpose:	The card is used for identification at the VA and for providing access to VA managed computer systems. Residents may not rotate at a VA site without a valid PIV card.

Renewal Information

The renewal process for PIV cards should be started 60 days before the expiration of the existing card.
